Identifying and Modifying Provider Ordering Behaviors Helps to Address Lab Over-Utilization and Low-Value Testing
Customer
Carle Health is a vertically integrated health system with a large physician group, three hospitals with more than 450 beds in the system, and a health plan. Carle’s lab supports both inpatient and outpatient practices.
Challenge
Like most hospital labs today, Carle’s lab faces the challenges of increasing test loads, shifting reimbursement models, and changing regulations.
Products
CareSelect™ Lab is a decision support tool that integrates with leading electronic health record (EHR) solutions. Its underlying clinical guidance includes more than 1,800 best practice rules and guidelines that are authored, curated, and maintained by Mayo Clinic physicians and scientists.
Results
The lab has responded to these challenges by implementing a lab stewardship program based on clinical decision support (CDS) to address lab over-utilization and low-value testing.
In the first 30 days after rolling out a non-interruptive intervention, Carle Health saw significant reductions in targeted orders:
- 39% decrease in inappropriate 1,25 dihydroxyvitamin D tests
- 77% reduction in inappropriate High-Sensitivity CRP tests
- Inappropriate ESRs dropped by 30% across the board, and by 70% in some departments
